Billing

We will gladly submit your claim to your insurance company. You will be responsible for any co-payments, coinsurance, and/or deductible obligations due. Co-payments are collected at the time of your appointment. We accept Visa and Mastercard in addition to personal checks and cash. If your insurance requires a referral and you do not have a referral (paper or electronic), you will be required to sign a referral waiver form and you will be responsible for the full charge for your care.
